Routine medical checkup
Plan Ahead Routine checkups (or physical exams) are performed by your primary care provider (PCP). You may change your PCP at any time for any reason. Appointments for routine checkups need to be made ahead of time. Routine checkups are scheduled weeks in advance.
If you are sick or injured
If you are ill and need a same-day appointment, call and describe your symptoms in detail. We reserve time each day for urgent care.
Dental
For a dental appointment, please call the Dental number at the Coatesville or Eastside Health Center. Dental check-ups and preventive cleanings and sealants should be scheduled in advance.
Arrive early for your appointment
Please arrive 15 minutes if you are a current patient and 30 minutes if you are a new patient. This will give you time to fill out any health history forms, make sure your contact information is up to date, and provide insurance information or apply for the sliding fee scale discount.
What to bring every visit
Your health insurance card(s), a valid photo ID, a list of your current medications and the co-pay required by your insurance. Other options would be to provide a passport or birth certificate. If you are uninsured, click here for information about our sliding scale fee discounts.
If you can’t keep your appointment
Please call us before the visit to let us know. The earlier you can call the better. If you need to cancel your appointment, we would appreciate 24 hours’ notice.
If you are late
It may not be possible for the provider to see you that day. Staff will work with you to reschedule.
