Arrive early for your appointment
Please arrive 15 minutes if you are a current patient and 30 minutes if you are a new patient. This will give you time to fill out any health history forms, make sure your contact information is up to date, and provide insurance information or apply for the sliding fee scale discount.
What to bring every visit
Your health insurance card(s), a valid photo ID, a list of your current medications and the co-pay required by your insurance. Other options would be to provide a passport or birth certificate. If you are uninsured, click here for information about our sliding scale fee discounts.
If you can’t keep your appointment
Please call us before the visit to let us know. The earlier you can call the better. If you need to cancel your appointment, we would appreciate 24 hours’ notice.
If you are more than 15 minutes late
It may not be possible for the provider to see you that day. Staff will work with you to reschedule.
